WIN A Tee SHIRT
Rules:
Each week, all patients who have been in the office that week will be entered into a drawing for a T-shirt. A name will be drawn from that pool of names for that week. Entries will then start over each week.
1. Only one entry per week. (Coming more often doesn’t increase your chances L of winning a T-shirt but it will make you feel better.)
2. Previous winners are ineligible to win again for 6 months.
3. You can “trade” your T-shirt prize for a cash equivalent in product or services. ($12 value.)
4. With your permission, you will have your picture taken with the T-shirt and be added to our winners board.
5. If we are out of your size, you will receive your shirt once a new order is received.
6. We respectfully ask that T-shirts be worn with pride and that given the opportunity you would gladly recommend our office to others.
